Rotary All Wheels Park on James Street - Donation Form

Thank you for choosing to donate towards the repair and expansion of our the Town of St. Marys skatepark. For more details about the project, please visit the Rotary All Wheels Park on James Street donation webpage.

Note: If you wish to be a named corporate sponsor, make an in-kind donation, have any questions or require a print version of this form, please contact Leslee Stacey at 519-284-2340, ext. 239 or lstacey@town.stmarys.on.ca. 

Collection of Personal Information

Personal information on this form is collected in accordance with the Municipal Freedom of Information and Protection of Privacy Act. It will be used for the purpose of tracking and promoting donations towards the Rotary All Wheels Park on James Street project. Any questions regarding the collection, use and retention of personal information on this form can be directed to Recreation Manager Darcy Drummond at ddrummond@town.stmarys.on.ca or 519-284-2340, ext. 626. 


 

Payment Options and Donation Levels

There are several payment options (select your option using the drop-down menu below).

Online with a credit card
Click "SUBMIT" at the bottom of this form to continue to the payment page. There is a maximum of a $1,000 donation with a credit card. For any donations over $1,000, please contact lstacey@town.stmarys.on.ca or 519-284-2340, ext. 239 to arrange an alternative payment method.

Online banking
Select St. Marys, Town of, General as payee and use first nine (9) digits of your phone number as the account number. Note: Each banking institution requires different criteria for the “payee name”. If you cannot find us, try searching St Marys (without punctuation) and look for the key word – General.

In person during operating hours
At Municipal Operations Centre (408 James Street South, St. Marys) from Monday to Friday, 8:30 a.m. to 4:30 p.m. Options include debit, cash or cheque. 

In person outside of operating hours
Please use the drop box at Municipal Operations Centre (408 James Street South). The drop box is to the left of the door. Only cheques accepted. Cheques should be made out to the Corporation of the Town of St. Marys with the following as the Memo/Subject Line: "St. Marys Skatepark".

By mail
Please mail cheques to:

Cheques should be made out to the Corporation of the Town of St. Marys with the following as the Memo/Subject Line: "St. Marys Skatepark".

Donation Receipts
A minimum $20 donation is required to receive a donation receipt for tax purposes. Once payment is received, and if a donation receipt is applicable, it will be sent to the mailing address you have provided through this form.

Donation Levels
You may be listed as a donor on our webpage (https://townofstmarys.com/allwheelspark) if your donation falls within one of the levels below and you consent to having your name displayed. Please note that it may take up to a week for your donation to be listed on our webpage.

  • $20 - $999 ~ FRIENDS
  • $1,000 - $9,999 ~ SUPPORTERS
  • $10,000 - $24,999 ~ HEROES
  • $25,000 - $49,999 ~ BENEFACTORS
  • $50,000 - $99,999 ~ CHAMPIONS
  • $100,000+ ~ FOUNDERS

A permanent acknowledgement will be included in the park for all donations at the Supporter level and above.
