Pet Tag Licence Form

About this Form:

Dog and cat owners are required by By-law 56-2023 to licence their dog and cat on an annual basis. For first time licences, please select 'new registration' below. For yearly renewals, please select 'renewal' listed below. Please note a seperate form is required for each animal.

By registering your dog or cat through this online form, you are helping to ensure that they are returned to you promptly should they ever get loose and be picked up by Animal Control. Beyond just peace of mind, the licensing fees also help maintain the dog park in St. Marys.

Personal information on this form is collected in accordance with the Municipal Freedom of Information and Protection of Privacy Act. It is to be used for the purpose of administering animal control practices. Any questions regarding the collection, use and retention of personal information on this form can be directed to the Town of St. Marys' Clerk (clerksoffice@town.stmarys.on.ca).

There a three payment options (select your option below):

• Online with a credit card: Click "SUBMIT" at the bottom of this form to continue to the payment page. 
  
  
• Online banking: Select St. Marys, Town of, General as payee and use first 9 digits of your phone number as the account number. Note that each banking institution requires different criteria for the “payee name”. If you cannot find us, try searching St Marys (without punctuation) and look for the key word – General.
  
  
• In person during operating hours: At Town Hall (175 Queen Street East) or Municipal Operations Centre (408 James Street South). Options include debit, cash or cheque.
  
  
• In person outside of operating hours:Drop box located at Town Hall, lower level, Church Street entrance. The drop box is on the right-hand side of the door. Only cheques accepted.

Once payment is received, the pet tag(s) will be mailed to the address provided in this form. Please be sure to include full mailing address including P.O Box number if applicable.
